Consumer Directed Services (CDS) is a Medicaid-funded program that allows eligible individuals to receive personal care services while remaining in control of their care decisions. Instead of relying on agency-hired caregivers, CDS empowers clients to hire, manage, and direct their own caregivers. This program is designed to help individuals who need assistance with daily tasks but still want the freedom to live independently in their homes. 

Who is Eligible for CDS?

To qualify for CDS, you must:

• Be Medicaid-eligible.
•  Over the age of 18
• Be able to self-direct your care, meaning you can hire and supervise a caregiver (which can be a trusted friend or family member).

What Services are Included in CDS?

Through the CDS program, you can receive support with:

• Personal Care: Assistance with bathing, grooming, dressing, and personal hygiene.
• Housekeeping: Help with cleaning, laundry, and maintaining a safe and tidy living environment.
• Meal Preparation: Assistance with planning and preparing meals to ensure proper nutrition.
• Transportation: Help with transportation for essential activities like medical appointments, grocery shopping, or social engagements.

The Independent Living Waiver (ILW) is a specialized Medicaid waiver program designed to provide additional services to individuals with significant needs that go beyond what is available through CDS. ILW services are aimed at helping clients remain in their homes and live independently for as long as possible. These services can be more comprehensive than those provided through the traditional Medicaid CDS program and are available to a limited number of eligible individuals.

Who is Eligible for ILW?

Eligibility for the Independent Living Waiver is determined based on:

• Medicaid eligibility.
• Meeting the physical disability requirements outlined by Medicaid.
• A need for services beyond those covered by CDS (e.g., more complex care, environmental modifications).

Due to the specialized nature of ILW services, there are a limited number of slots available. Our team will work with you to determine your eligibility and assist with the application process to ensure you can access the care you need.

What Services are Included in ILW?

The ILW program offers a broader range of services to meet more complex needs, including:

• Case Management: Personalized guidance and support to coordinate your care and ensure you have access to all the necessary services and resources.
• Personal Care: Similar to CDS, but with enhanced services tailored to those with higher needs.
• Environmental Accessibility Adaptations: Home modifications to make your living space more accessible and safe, such as installing wheelchair ramps, handrails, or widening doorways.
• Specialized Medical Equipment & Supplies: Access to medical equipment and supplies that help you manage your health and maintain independence, such as hospital beds, mobility aids, or respiratory devices.
